Our discharge planning begins from time of admission. Throughout treatment, our goal is to identify and fix all the factors that have led to the eating disorder and may lead to relapse once the resident is discharged. Hence, a mindful, cautious, and gradual process characterizes discharge planning.
From the time of admission and throughout treatment, we maintain close communication with the referring therapist or physician. The purpose of this is two fold: one, it gives us greater insight and background when developing the treatment plan; and two, it establishes a continuity of care at time of discharge. We recommend follow up after discharge with the referring doctor. Should the individual choose not to follow with the referring physician or therapist or did not have a pre-admission therapist, we will refer them to one at time of discharge.
Depending on the individual's circumstance, we often recommend continued family therapy in order to strengthen the home environment and prevent relapse.
Residents are encouraged to maintain frequent phone contact with staff following discharge to help remind them of the treatment principles as well as help them with their struggles and issues as they arise. Also, weekly contact with our staff dietitian is encouraged for up to one year following discharge.
Finally, Montecatini hosts an annual Alumni Day, in which individuals re-visit with staff and current residents, as well as reconnect with the treatment strategy and strengthen their recovery.
